Pathways

 

Pathways is designed to reduce unmet needs, address health inequities, and improve the overall health of the residents of Bernalillo County.

Pathways Mission and Goals:

 

•   Connecting underserved county residents with health care and other support systems and assisting them as     they navigate through it;

•   Coordinating services for the underserved residents to achieve positive individual-level health outcomes;

•   Assuring collaborative planning and improvement of our health care system.

 

Our Pathways navigator Monica Asencio and our two interns, Irene Diaz and Laura Price-Walderman from New Mexico Highlands University are helping our patients and clients to connect with appropriate services in order to improve their health outcomes.  

 

Our most recurrent pathways are:

 

· Housing,

· Dental care

· Driver’s license/I.D

· Education/GED

· Employment

· Medical debt

 

Our most difficult systems to navigate

 

· Housing (public housing)

· Public health systems

· Driver’s license/I.D

 

This fiscal year we are serving Pathways clients

· We are currently helping 19 clients. 

· We have 3 clients on the waiting list.

 

 

· In 2014, 21 clients have graduated from the Pathways Program. 

Our People


Monica Asencio joined Casa de Salud as a Health Navigator in June 2014, after receiving her Master’s degree from the University of New Mexico, where she studied Health Education. Monica works with both the Spanish and non-Spanish speaking at-risk populations by helping them find primary care providers and the resources they may need. Monica’s main goal is to improve people’s health and reduce health inequalities that disproportionally affect people due to income, race, and other factors.
